MOWW Information Update Form
Please follow the instructions below to submit your information updates.
Form 5 is used for an individual looking to update their own membership information.
Form 6 is for Chapter Officer information updates to the National Directory.
Form 6 may also be used to update National Officers such as Board or Committee members. Please note that the Directory only lists Chair and Vice Chair, so the choices are available for One or Two Officers only.
MOWW Form 5: Membership Information Update
Please fill out all fields that need to be updated.
Please note that new rosters are uploaded at the end of each week, and the end of each month (if not a Friday). If you submit any changes, please wait until after the next upload to re-submit. It may take a few days before the next roster is uploaded.

Please include all updates. If an officer did not change or has no updates, please leave blank.
Please note that after submission it may take a few days to update the National Directory.
